Submit a Claim - Claim Instructions

How to File Online:

Before Claim Filing:

Unique ID and PIN: You will need the Unique ID and PIN to submit a claim online. Each Class Member’s Unique ID and PIN are included in the Email Notice and/or Postcard Notice. If you did not receive the Notice, you can submit a claim by mail. A copy of the Claim Form is available on the Documents page.

During Claim Filing:

If you are a Settlement Class Member, you can submit a Claim Form for the following Settlement Class Member Benefits:

Cash Payment A – Documented Losses: You may submit a Claim Form and provide reasonable documentation for out-of-pocket losses related to the Data Incident for up to $5,000.00 per Settlement Class Member;

Cash Payment B – Pro Rata Cash: You may also submit a Claim Form, without providing documentation, to receive a pro rata (a legal term meaning equal share) Cash Payment in the estimated amount of $75; AND

If you elect to receive Cash Payment A – Documented Losses, you must also submit reasonable documentation for losses related to the Data Incident.

Credit Monitoring: In addition to Cash Payment A and Cash Payment B, you may also submit a Claim Form to receive two years of one bureau Credit Monitoring.

Examples of reasonable documentation include (but are not limited to): telephone records, correspondence including emails, or receipts. Personal certifications, declarations, or affidavits from you do not constitute reasonable documentation but may be included to provide clarification, context, or support for other submitted reasonable documentation. You will not be reimbursed for expenses if you have been reimbursed for the same expenses by another source, including compensation provided in connection with the credit monitoring and identity theft protection product offered as part of the notification letter provided by Dartmouth or otherwise.

If you do not submit reasonable documentation supporting a loss, or if your Claim Form is invalid as determined by the Settlement Administrator, and you do not cure your Claim Form, your Claim Form will be processed as if you selected the Cash Payment B - Pro Rata Cash claim, if you submitted a timely and valid Claim for for Cash Payment B.

Confirmation Code:

After submitting your completed claim online, you will receive an email with a confirmation code for your completed submission. Be sure to keep your confirmation email and code and refer back to them if you have any questions about your Claim Form.

Remember, all Claim Forms must be submitted online or received by the Settlement Administrator no later than October 21, 2026. Late submissions will not be accepted.
